How exactly do you insert data? maybe there is a BEGIN but no COMMIT?
How exactly dou you verify that it does not exist?

A plenty of reasons is possible, starting from uncommited transactions 
on shutdown till invalid fsync method or hardware problem (i.e. failed 
accu on Adaptec HW raid cache).
Some more information is required to study your case, at least OS / 
config / hardware info.
