more doc cleanup

This patch fixes a few missed GUC variables that were still upper case,
makes a few more small improvements to runtime.sgml, and makes some SGML
conventions more consistent.

Right (... although I always build a copy of the docs locally while I'm
making changes, to double-check that they still build).

BTW, do you mean that I should use developer.postgresql.org in
preference to the autobuilt docs @

No, use that URL.  I just point to the bottom of
http://developer.postgresql.org/index.php because it has the log file
and the build output.
