Obsolete debug #define in pg_config_manual.h

Attached patch removes obsolete debugging #define in
pg_config_manual.h (RTDEBUG).

This was last used in commit 2a8d3d, from 2005. Apparently RTDEBUG is
a contraction of "R-Tree Debug".
