VACUUM failing ??!!

After running for more than 24 hrs (I have a large db), vacuum failed with
this message:
ERROR:  failed to re-find parent key in "pg_type_typname_nsp_index"
Has anybody encountered this before?

AFAIR, you have to REINDEX pg_type to get rid of this error.
