SELECT *
  FROM  somewhere
  WHERE lastname !~ '[A-Z]' AND lastname !~ '[a-z]';

will INCLUDE the fields that *don't* start with A-Za-z  while

SELECT *
  FROM  somewhere
  WHERE lastname ~ '[A-Z]' AND lastname ~ '[a-z]';

will EXCLUDE the fields that *don't* start with A-Za-z
