Differences from v76* are:
* Rebased to HEAD @ yesterday
* v77* addresses most of Amit's recent feedback comments [1]; I will
reply to that mail separately with the details.
* The v77-003 is temporarily omitted from this patch set. That will be
re-added in v78* early next week.
