Hmm.  I wonder if log_newpages() is confused here:

    XLogEnsureRecordSpace(XLR_MAX_BLOCK_ID - 1, 0);

Why is XLR_MAX_BLOCK_ID - 1 enough, rather than XLR_MAX_BLOCK_ID?
