The typical advice is to run CREATE INDEX CONCURRENTLY instead of plain
CREATE INDEX.  Also, use "ADD CONSTRAINT ... NOT VALID;" instead, and
later do another ALTER TABLE ... VALIDATE CONSTRAINT.  That decreases
the impact considerably in production scenarios.
