They have not been reviewed.

If it's a matter of either-or I'd really prefer one page per grouping over getting rid of the table-of-contents.  But I suspect there has to be some way to add an sgml element to the markup to force a new page and would prefer to confirm or refute that prior to committing 0006.

0005 seems a win either way though I haven't reviewed it yet.
