Certainly, this is something I have received as a comment for couple of times during the test with my internal team.

Have a look at the below updated mockup now -

Changes done - 
- I have taken off the dedicated check boxes for 'Revoke'
- Combined 'Grant and Revoke' in single check box where it has 3 state in it
- Like Grant (If checked), Revoked (If cross) and None (If unchecked)

Do you think the mockup satisfies all the conditions now?  Just share your opinion.
