Here is my pg_hba

local all trust
host all 127.0.0.1 255.255.255.255 trust
host all 172.16.134.0 255.255.255.0 password
pg_passwd
host all 123.123.123.0 255.255.255.0 password
pg_passwd
host all 0.0.0.0 0.0.0.0 reject

This allow local (trusted) connections, and connections from 172.16.134.*
and 123.123.123.* networks (with passwords).
all othe is rejected

Local trusted connection is not good idea if you have many local users... niki
