Changes applied;  sorry for the delay. I had to create a new patch
because the submitted patch had invalid markup, moved the first
paragraph of items but left the second paragraph alone, and I didn't
like the order of the error_detail item.  I ended up moving the error
detail item to Source Code, and swapped the validator and hook items.
