I went through this and made some mostly-cosmetic changes. I think the attached v3 is ready to go, but I'll wait a day or so to see if anyone has any comments.

I just saw v3 after sending the review comments for v2. Looks like my comment 1 has been addressed in v3, and the comment 2 still stands.
