I used this successfully in the past with postgis:
postgis_libdir := $(shell pg_config --pkglibdir)
postgis_libver := $(shell ls -1 $(postgis_libdir) | grep "^postgis")
SHLIB_LINK += -L$(postgis_libdir) -l:$(postgis_libver) -llwgeom
