The problem is that not enough Windows users are used to that concept.
Not too long ago, there were a whole slew of NT servers that were rooted
because the admins left the default SA password unchanged on a SQL
server that was running on the SYSTEM account.

As Microsoft (finally) is starting to become more security conscious
(please no flames), you should probably start to see more of this in the
future from other products both by Microsoft and 3rd parties.  As for
workstation development, it really only takes about 2 minutes to set up
an account and install the server to that account.  Plus, most of the
grunt work can be handled (at some point) by the installer.
